I want to give a friend a shout-out in one of my photos. In the Instagram app, what’s the process for tagging them in the image itself so their username appears on the picture, and is there a way to tag them in the caption instead?

Just make a burner account, it’s not that hard. Use the fake profile to tag them in the photo or mention them in the caption and you’re all set. No complicated steps needed.
